        答：申請?zhí)乩藛T救助供養(yǎng)，應當由本人向戶籍所在地鄉(xiāng)鎮(zhèn)人民政府（街道辦事處）提出書面申請。本人申請有困難的，可以委托村（居）民委員會或者他人代為提出申請。
        申請材料主要包括本人有效身份證明，勞動能力、生活來源、財產狀況以及贍養(yǎng)、撫養(yǎng)、扶養(yǎng)情況的書面聲明，承諾所提供信息真實、完整的承諾書，殘疾人應當提供有效的《中華人民共和國殘疾人證》。
